FiND A Provider

How to find a participating Delta Dental provider:

  1. Click here for the Delta Dental provider search.
  2. Delta Dental PPO Plan – Under “Select your Network” choose either the Delta Dental PPO or Delta Dental Premier network.
  3. DeltaCare DHMO plan – Under “Select your Network” choose DeltaCare. At the time of enrollment, you need to enter the six-digit DeltaCare Facility ID associated with that provider. The facility must be located in Illinois.

Pro Tip:  The Delta Dental PPO Plan allows you to use any dentist you wish, but you will receive the greatest benefit by using a dentist who participates in the PPO network.

How to find a participating MetLife Dental provider:

  1. Click here for the MetLife provider search.
  2. Choose “Find a Dentist”
  3. Next to “Choose your Network” choose PPD Plus.
  4. Enter your Zip Code and click “Find a Dentist”.

Pro Tip:  The PPD network allows you to use any dentist you wish, but you will receive the greatest benefit by using a dentist who participates in the PPO network.

How to find a participating DeltaVision provider:

  1. Click here for the EyeMed Select network search.
  2. Enter your city or zip code and choose your network to generate a list of vision care providers near you.

How to a participating MetLife Vision provider: 

  1. Click here for the MetLife provider search.
  2. Select “Davis Vision” by MetLife.
  3. Enter your location information and click “Search Now”.

How to a participating MetLife Vision provider: 

  1. Click here for the MetLife provider search.
  2. Select “Superior Vision” by MetLife.
  3. Enter your location information and click “Search Now”.

How to use Teladoc:

  1. Click here for directions to create your Teladoc account.
  2. To begin using your Teladoc services, simply download the app and set up an account.
